1. I Am Authorizing The Following Procedures: Sedation, General Anesthesia, Feline Neuter, Pain medication Injection, Nail Trim, Tattoo, Pain Medication To Go Home.

2. I understand that there will be no pre-anesthetic bloodwork performed for my pet prior to sedation, and accept the potential added risk posed to my pet as a result. Risks included but are not limited to; no IV fluids, No heart rate/rhythm monitor, No blood pressure monitoring, no expired CO2 monitoring.

3. I understand there will be an RVT performing manual HR, RR and temperature monitoring only. I understand my cat will be recovered in a group setting with one RVT monitoring and recovering several patients at one time.

4. I understand and accept the risks associated with General anesthesia. While anesthesia is generally safe, there are inherent risks and potential complications associated with its use. These risks may include but are not limited to: Variations in heart rate, respiratory rate, blood pressure, and temperature, allergic reaction and possible death.

I understand that during this procedure minimal life saving measures will be deployed should an anesthetic incident occur (ie. Anesthetic agents may be reversed however chest compressions will NOT be attempted).
